Members with diabetes get customized interventions across all five clinical impact areas:

  1. Lifestyle and comorbidity management. Nutrition and exercise; weight management; hypertension monitoring; tobacco cessation; and more
  2. Medication optimization or reduction. Working with a member’s PCP to add or change diabetes and hypertension medications when needed
  3. Monitoring blood glucose. Self-monitoring and continuous monitoring solutions with options to keep costs low for you and for them

De-prescribing
